If you choose the Monthly Flat Fee option, I will take your credit card information or debit card information. I will then set up a recurring charge on the account. Again, you can cancel at anytime. I just need a 30 day notice in writing of your cancelation. Remember if you wish to sign up again for the Monthly Flat Fee option later in the future, there is a $99 re-registration fee per patient.
